Enrollment

Woodbury Lutheran's enrollment process is based on a priority system. Enrollment forms are accepted and processed in the following order prior to January 20, 2010. After January, enrollment is accepted on a first-come, first-served basis.

The checklist below has been designed to assist you as you go through the enrollment process.

I. Getting To Know Us

* Visit our website at www.woodburylutheran.org. Click on School.

* Call to speak to the Director and/or schedule a tour of our facility or attend an Open House.

* Obtain enrollment forms from our school office any weekday, to request forms, or download at the following links.

II. Complete and Return Enrollment Form and Fee.

* Submit fully completed form with non-refundable registration fee to our school office or mail to:

*Online enrollment is not accepted.

* A student information packet and confirmation will be mailed to you within 2-4 weeks of receiving your form & fee. Please file this packet of student information forms in a safe and easily accessible place!

III. Return Completed Student Information Forms Below On Dates Requested (August 1/Preschool and June 1/Kindergarten)

* Child Personal Information forms/ * Release / Consent Forms/ * Signed Health Care Summary and Immunization Form/ * Child Emergency Card

IV. Call to schedule an appointment with the Director to discuss Allergies or Health Concerns.

* We want to be educated regarding your child's special needs in order to insure a safe, healthy, and positive early learning experience. Important forms must be completed by your child's health care provider.

V. Additional Forms below will be sent prior to the start of school, given at Orientation in the fall, or at time of late enrollment.
